【摘要】：目的采用經陰式三維超聲評價足月妊娠經陰道分娩中應用外源激素對盆底功能的影響。方法 90名足月經陰道分娩的產婦按使用外源激素情況分為自然分娩組、催產素組和前列腺素E2(PGE2)組,各30名。于產后42天進行超聲檢查,分別獲得靜息狀態(tài)、瓦氏和縮肛動作時盆隔裂孔圖像,比較3組產婦盆隔裂孔前后徑和面積。結果自然分娩組和催產素組于靜息、瓦氏和縮肛動作時盆隔裂孔的前后徑、面積測量值差異均無統(tǒng)計學意義(P均0.05)。PGE2組與自然分娩組、催產素組在靜息、瓦氏和縮肛動作時盆隔裂孔測量值差異均有統(tǒng)計學意義(P均0.05)。結論足月妊娠經陰道分娩中應用不同外源激素的產婦產后盆底功能存在差異,應用外源PGE2對盆底功能的損傷大于應用催產素和自然陰道分娩;應用催產素對盆底功能的影響較自然陰道分娩無差異。
[Abstract]:Objective to evaluate the effects of exogenous hormones on pelvic floor function in term pregnancy during vaginal delivery by transvaginal three-dimensional ultrasound. Ninety women with term vaginal delivery were divided into natural delivery group according to the use of exogenous hormones. In the oxytocin group and prostaglandin E _ 2 PGE _ 2 group, 30 patients were examined by ultrasound on 42 days postpartum, and the images of pelvic septum were obtained in resting state, Vapor and anal contraction respectively. Results the anteroposterior diameters of pelvic septal holes in the spontaneous delivery group and oxytocin group were compared. There was no significant difference in the area measured values between the two groups (P < 0.05) and the natural delivery group, and the oxytocin group was resting. There were significant differences in the measurement value of pelvic septum between Vapor and anal contraction (P < 0.05). Conclusion there are differences in pelvic floor function between pregnant women who use different exogenous hormones during vaginal delivery during term pregnancy. The damage to pelvic floor function by exogenous PGE2 was greater than that by oxytocin and natural vaginal delivery. The effect of oxytocin on pelvic floor function was not different from that of natural vaginal delivery.
