【摘要】：目的:運用自編問卷的方法,探究不同證候表征方式對中醫(yī)學生辨證診斷正確率的影響及中醫(yī)學生對陽明病癥狀群落的認知情況,從而研究癥狀群落思維的表征規(guī)律及其對中醫(yī)學生辨證診斷思維的影響。方法:1、文獻研究:梳理總結“病”、“證”、“證候”、“癥”、“病機”等中醫(yī)基本概念及其關系;探索中醫(yī)辨證思維規(guī)律的研究現(xiàn)狀及發(fā)展。2、實證研究:采用隨機整群抽樣的方法,抽取2013級中醫(yī)學七年制專業(yè)本科生以及2014級中醫(yī)學研究生共251名作為調查研究對象,發(fā)放自編問卷《不同證候表征方式對陽明病證候診斷正確率的影響調查問卷》,回收后進行數(shù)據(jù)統(tǒng)計分析。結果:1、總體來看,癥狀群落表征模式證候診斷的正確率最高,隨機自由表征模式證候診斷的正確率最低。2、在癥狀群落表征模式上,研究生證候診斷的正確率要顯著高于本科生,女生證候診斷的正確率要顯著高于男生。3、中醫(yī)學專業(yè)學生對陽明病本證的總體掌握情況要明顯好于變證,其中對陽明燥熱實邪內結證掌握地最好。結論:1、中醫(yī)學專業(yè)學生對知識掌握的程度以及自身的臨床經驗對辨證診斷正確率有重要影響。2、癥狀群落表征模式注重對雜亂無序的癥狀進行基本病機的分類表征,有利于中醫(yī)學專業(yè)學生在對病機綜合分析的基礎上提高辨證診斷正確率。
[Abstract]:Objective: to explore the effect of different syndromes representation methods on the correct rate of syndrome differentiation and diagnosis of TCM students and the cognition of TCM students to the symptom community of Yangming disease by means of self-made questionnaire. Thus, the characteristics of symptom community thinking and its influence on TCM students' thinking of syndrome differentiation and diagnosis were studied. Methods: 1, literature research: combing and summarizing the basic concepts of TCM, such as "disease", "syndrome", "pathogenesis" and so on; To explore the current situation and development of TCM dialectical thinking. 2. Empirical study: 2013 grade Chinese medicine seven-year undergraduate and 2014 grade Chinese medicine graduate students were selected by random cluster sampling method as the objects of investigation. The effect of different syndromes on the correct rate of diagnosis of Yangming disease was investigated. The data were collected and analyzed. Results: 1. On the whole, the correct rate of symptom community representation pattern was the highest, and that of random free representation model was the lowest. 2. In symptom community representation model, the correct rate of graduate student syndrome diagnosis was significantly higher than that of undergraduate students. The correct rate of syndrome diagnosis of female students was significantly higher than that of boys. 3. The overall mastery of Yangming disease in TCM students was obviously better than that of changing syndrome, among which, the internal syndrome of Yang Ming dryness and heat evil was the best. Conclusion: 1. The degree of knowledge mastery and their own clinical experience in TCM students have an important influence on the correct rate of diagnosis of syndrome differentiation. 2. The symptom community representation model focuses on the classification and characterization of the basic pathogenesis of the disorder symptoms. It is beneficial for the students of TCM to improve the correct rate of syndrome differentiation and diagnosis on the basis of comprehensive analysis of the pathogenesis.
