【摘要】：目的:分析鉤突上端附著類型與慢性額竇炎不伴鼻息肉的相關(guān)性。方法:觀察240側(cè)額隱窩薄層CT中的鉤突上端附著類型,比較其在慢性額竇炎不伴鼻息肉組與正常對(duì)照組之間的差異,分析其與慢性額竇炎不伴鼻息肉的相關(guān)性。結(jié)果:鉤突上端附著類型有7種,最常見的為1型(鉤突上端附著于眶紙板)。單因素分析顯示,1型鉤突上端附著方式在慢性額竇炎組的出現(xiàn)率高于對(duì)照組(59.8%、36.4%,P0.01),5型鉤突上端附著方式(鉤突上端同時(shí)附著于眶紙板及前顱底)在慢性額竇炎組的出現(xiàn)率低于對(duì)照組(9.8%、28.8%,P0.01)。多因素Logistic分析表明,1型鉤突上端附著方式是慢性額竇炎的危險(xiǎn)因素(OR=1.181,P0.05),5型鉤突上端附著方式是慢性額竇炎的保護(hù)因素(OR=0.391,P0.05)。結(jié)論:鉤突上端附著方式變異較大,最常見的類型是1型;當(dāng)鉤突上端附著于眶紙板時(shí)易患慢性額竇炎不伴鼻息肉。
[Abstract]:Objective: to analyze the relationship between the type of upper end attachment of uncinate process and chronic frontal sinusitis without nasal polyp. Methods: the types of superior uncinate attachment in thin layer CT of frontal recess were observed and compared between the chronic frontal sinusitis without nasal polyp group and the normal control group, and the correlation between them and chronic frontal sinusitis without nasal polyps was analyzed. Results: there were 7 types of attachment to the upper end of the uncinate process, the most common type was type 1 (the upper end of the uncinate process attached to the orbital cardboard). Univariate analysis showed that the occurrence rate of type 1 upper end attachment of uncinate process in chronic frontal sinusitis group was higher than that in control group (59.8% 36.4% P0.01). The incidence of type 5 upper end attachment of the uncinate process (the upper end of the uncinate process attached to the orbital board and the anterior skull base) in the chronic frontal sinusitis group was lower than that in the control group (9.828% P 0.01). Multivariate Logistic analysis showed that type 1 superior uncinate attachment was a risk factor for chronic frontal sinusitis (OR=1.181,P0.05), and type 5 was a protective factor for chronic frontal sinusitis (OR=0.391,P0.05). Conclusion: the type 1 is the most common type, and the chronic frontal sinusitis without nasal polyp is easy when the upper end of the uncinate process is attached to the orbital board.
