【摘要】：目的探究繼續(xù)動(dòng)機(jī)性訪談對(duì)幽門(mén)螺桿菌感染患者遵醫(yī)依從性。方法選取2013年1月-2016年1月于醫(yī)院接受治療的448例幽門(mén)螺桿菌感染患者為研究對(duì)象,按照隨機(jī)數(shù)表法將患者均分為觀察組和對(duì)照組,各224例;對(duì)照組實(shí)施常規(guī)幽門(mén)螺桿菌感染干預(yù),觀察組在對(duì)照組基礎(chǔ)上加以繼續(xù)動(dòng)機(jī)性訪談,通過(guò)意圖前期、意圖期、準(zhǔn)備階段、行動(dòng)階段以及維持階段對(duì)患者加以干預(yù);干預(yù)1、3、6個(gè)月后,比較兩組患者幽門(mén)螺桿菌根除率;采用醫(yī)院自制的遵醫(yī)依從性量表對(duì)兩組患者包括遵醫(yī)服藥次數(shù)、服藥劑量、間斷停藥、服藥時(shí)間、合理飲食、運(yùn)動(dòng)鍛煉六個(gè)維度進(jìn)行評(píng)定;采用自我管理量表對(duì)兩組患者食物選擇、規(guī)律鍛煉、用藥管理、營(yíng)養(yǎng)監(jiān)測(cè)、進(jìn)餐方式以及戒煙戒酒等自我管理能力進(jìn)行比較分析。結(jié)果干預(yù)1、3、6個(gè)月后,觀察組幽門(mén)螺桿菌根除率分別為18.8%、54.9%和79.0%,對(duì)照組分別為9.3%、35.7%和66.5%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);兩組患者遵醫(yī)依從性量表評(píng)定結(jié)果顯示,觀察組遵醫(yī)服藥次數(shù)、服藥劑量、間斷停藥、服藥時(shí)間、合理飲食、運(yùn)動(dòng)鍛煉的依從性均高于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05);患者自我管理能力比較結(jié)果顯示,觀察組患者各項(xiàng)自我管理指標(biāo)水平均高于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論繼續(xù)動(dòng)機(jī)性訪談能夠提高幽門(mén)螺桿菌感染患者遵醫(yī)依從性,對(duì)于提高患者幽門(mén)螺桿菌根除率、降低患者感染率具有重要意義,可在臨床推廣應(yīng)用。
[Abstract]:Objective to explore the compliance of patients with Helicobacter pylori infection with continuing motivation interviews. Methods 448 patients with Helicobacter pylori infection who were treated in hospital from January 2013 to January 2016 were divided into observation group (n = 224) and control group (n = 224). In the control group, the routine Helicobacter pylori infection intervention was carried out, and the observation group continued the motivation interview on the basis of the control group, and intervened the patients through the preintention, the intention period, the preparation stage, the action stage and the maintenance stage. The rate of Helicobacter pylori eradication was compared between the two groups after intervention 1 ~ 3 and 6 months, and the hospital self-made compliance scale was used to treat the patients in the two groups, including the number of times of compliance, dosage of medication, intermittent withdrawal of drugs, time of taking medicine, and reasonable diet. Six dimensions of exercise were evaluated, and the self-management abilities such as food selection, regular exercise, medication management, nutrition monitoring, eating style and quitting smoking and drinking were compared between the two groups by self-management scale. Results the eradication rate of Helicobacter pylori in the observation group was 18.8% and 79.0%, respectively, and that in the control group was 9.33.5% and 66.5%, respectively. The difference between the two groups was statistically significant (P 0.05). The compliance of the two groups was higher than that of the control group (P 0.05), and the results showed that the patients' self-management ability was higher than that of the control group (P < 0.05). The level of self-management indexes in the observation group was higher than that in the control group, and the difference between the two groups was statistically significant (P 0.05). Conclusion continuous motivation interview can improve compliance of patients with Helicobacter pylori infection, improve the eradication rate of Helicobacter pylori and reduce the infection rate of patients. It can be popularized and applied in clinical practice.
