【摘要】：目的探討烏司他丁(UTI)對重癥急性胰腺炎(SAP)大鼠腸黏膜屏障的影響及其作用機制。方法 SD大鼠60只分成對照組、假手術(shù)(SO)組、SAP組和UTI組。制模后每隔8 h進行腹腔內(nèi)注射UTI(濃度1.5萬IU/kg)作為UTI組。各治療組檢測制模后8、24、72 h血漿內(nèi)毒素水平,通過RT-PCR法和免疫組化法對腸黏膜Toll樣受體(TLR)4表達進行檢測,并在電鏡下觀察腸黏膜組織。結(jié)果對照組腸黏膜未見TLR4表達。與對照組相比,SAP組在制模后8 h血漿內(nèi)毒素和腸黏膜TLR4表達即出現(xiàn)升高,并隨時間的延長而繼續(xù)進行性增高。血漿內(nèi)毒素與腸黏膜TLR4二者明顯相關(guān)。UTI組同時點內(nèi)毒素、TLR4低于SAP組,電鏡下檢查發(fā)現(xiàn)病理損傷也同時減輕,24 h時最為明顯。結(jié)論 UTI能有效保護SAP發(fā)生時的腸屏障功能,其保護機制可能與腸黏膜TLR4的表達下調(diào)有關(guān)。
[Abstract]:Objective to investigate the effect and mechanism of ulinastatin (UTI) on intestinal mucosal barrier in rats with severe acute pancreatitis (SAP). Methods 60 SD rats were divided into control group, sham operation group and UTI group. The UTI group was treated by intraperitoneal injection of UTI (15000 IU / kg) every 8 hours. The plasma endotoxin level was measured at 824 hours after model making in each treatment group. The expression of Toll like receptor TLR4 in intestinal mucosa was detected by RT-PCR and immunohistochemistry, and the intestinal mucosal tissue was observed under electron microscope. Results there was no TLR4 expression in intestinal mucosa of control group. Compared with the control group, plasma endotoxin and intestinal mucosal TLR4 expression increased at 8 h after model making, and continued to increase progressively with the prolongation of time. There was a significant correlation between plasma endotoxin and intestinal mucosal TLR4. TLR4 in UTI group was lower than that in SAP group at the same time. Conclusion UTI can effectively protect the intestinal barrier function during the occurrence of SAP, and its protective mechanism may be related to the down-regulation of TLR4 expression in intestinal mucosa.
