護理本科實習(xí)生心理健康狀況及相關(guān)因素的縱向研究

【摘要】: 目的:通過不同時間點對本、專科護生應(yīng)激性事件及心理健康狀況進行跟蹤測評,探討護理本科實習(xí)生心理狀況變化的特點及其客觀影響因素,為及時發(fā)現(xiàn)護生的心理問題、有的放矢地對其進行心理健康教育和心理問題的干預(yù)提供科學(xué)依據(jù)。 方法:①采用隨機抽樣的方法,抽取湖南、山東兩地兩所醫(yī)學(xué)院校護理專業(yè)2007屆畢業(yè)生共295人,本科組165人和專科組130人。采用大學(xué)畢業(yè)生應(yīng)激性事件量表(CSSS)和癥狀自評量表(SCL-90)進行5次歷時近8個月的跟蹤調(diào)查。所得數(shù)據(jù)采用SPSS13.0軟件包,應(yīng)用兩樣本t檢驗分析護生心理健康狀況與全國青年常模的差異,應(yīng)用重復(fù)測量的方差分析、SNK-q檢驗等方法,分析本、?迫巳盒睦斫】禒顩r的變化特點;應(yīng)用x~2檢驗分析本?谱o生不同時點心理問題發(fā)生率的差異;②采用Pearson相關(guān)分析及逐步回歸,分析實習(xí)不同階段應(yīng)激性事件頻率變化與心理健康狀況變化的關(guān)系。 結(jié)果:①兩樣本t檢驗結(jié)果顯示,本科護生在實習(xí)期間不同測試時點,,SCL-90各因子分低于全國18~29歲青年常模(p<0.05或0.01)或與全國青年常模比較差異無統(tǒng)計學(xué)意義(p>0.05);②x~2檢驗結(jié)果顯示,在T5、T7時點?粕男睦韱栴}發(fā)生率高于本科生(p<0.05);③重復(fù)測量方差分析結(jié)果顯示,不同測試時點護生的心理健康狀況不同(p<0.05),本科組與?平M之間存在組間差異,并且分組與時間之間存在交互作用;④兩樣本t檢驗結(jié)果顯示,本科護生與專科生在T0時點與T1時點SCL-90總分差異無統(tǒng)計學(xué)意義(p>0.05),而在T3、T5、T7時點,SCL-90總分本科組低于?平M(p<0.05);⑤q檢驗結(jié)果表明,本科護生在T3時點SCL-90總分最低,T3時點與T0、T1、T5、T7之間差異均有統(tǒng)計學(xué)意義(p<0.05),而T0、T1、T5、T7兩兩之間差異無統(tǒng)計學(xué)意義(p>0.05);專科護生T5、T7時點SCL-90總分與T0、T1、T3之間兩兩比較,差異有統(tǒng)計學(xué)意義(p<0.05),T5與T7之間,以及T0、T1、T3兩兩之間SCL-90總分之間的差異無統(tǒng)計學(xué)意義(p>0.05);⑥Pearson相關(guān)分析結(jié)果顯示,在實習(xí)的不同階段,大多數(shù)應(yīng)激性事件頻率的變化與SCL-90總分變化之間存在顯著正相關(guān)(p<0.05);⑦逐步回歸結(jié)果顯示,第一階段,人際關(guān)系的變化進入了回歸方程(p<0.01);第二階段,人際關(guān)系壓力及日常生活困擾的變化依次進入了回歸方程(p<0.01);第三階段,人際關(guān)系壓力及學(xué)習(xí)壓力依次進入了回歸方程(p<0.01或0.05);第四階段,求職壓力與日常生活困擾依次進入了回歸方程(p<0.01或0.05)。 結(jié)論:①與全國青年常模相比,本科護生在不同測試時點心理健康狀況較好;②在不同的測試時點,本?谱o生心理健康狀況不同,本科護生好于?粕虎郾究茖嵙(xí)護生從實習(xí)前兩周到實習(xí)第七個月末,心理健康水平逐漸上升又逐漸下降至與實習(xí)前水平持平,而?谱o生的心理健康水平先保持平穩(wěn)又逐漸下降且低于實習(xí)前水平;④不同實習(xí)階段影響護生心理健康狀況變化的因素不同;⑤重視護生實習(xí)前與擇業(yè)期的心理健康教育,幫助護生正確處理人際關(guān)系,使其確立正確的職業(yè)生涯規(guī)劃。
[Abstract]:Objective: To evaluate the stressful events and mental health status of the nursing students at different time points, to explore the characteristics and objective factors of the changes in the psychological status of the nursing undergraduates, and to find out the psychological problems of the nursing students in time and to carry out the psychological health education and psychological problems in a definite direction. Learn the basis.
Methods: (1) a total of 295 nursing students from two medical colleges and universities in Hunan and Shandong were selected by random sampling, and 165 students and 130 college students in the undergraduate group were enrolled in this study. The students' stress event scale (CSSS) and the symptom checklist (SCL-90) were used to carry out a follow-up survey of 5 times for nearly 8 months. The data were used in SPSS13.. 0 software package, using two samples t test to analyze the difference between the mental health status of the nursing students and the National Youth norm, the analysis of variance and SNK-q test of repeated measurement, the characteristics of the psychological health status of this special group, and the difference of the incidence of psychological problems at different points of the nursing students by x~2 test; and the use of Pe Arson correlation analysis and stepwise regression analysis were conducted to analyze the relationship between frequency of stressful events and mental health in different stages of internship.
Results: (1) the results of two samples t test showed that there was no significant difference between the SCL-90 factor scores of the national 18~29 year old youth norm (P < 0.05 or 0.01) or the National Youth norm (P > 0.05) during the different test time points during the practice period of undergraduate nursing students (P > 0.05). (2) the result of x~2 test showed that the psychological problems of junior college students were higher in T5 and T7. The results of the repeated measurement of variance analysis showed that the psychological health of the nursing students at different time points was different (P < 0.05), and there was a difference between the group and the specialist group, and there was a interaction between the group and the time. (4) the t test of two samples showed that the undergraduate nursing students and the junior college students were at the time point of T0 and the time point of T1. The total score of SCL-90 was not statistically significant (P > 0.05), while in T3, T5 and T7, the total score of SCL-90 in the undergraduate group was lower than that of the specialist group (P < 0.05). 5. The results of Q test showed that the total score of SCL-90 in the undergraduate nursing students was the lowest at T3 time point, and there were statistical significance between T3 time points and T0, wasting, and 22. P > 0.05); the difference was statistically significant (P < 0.05), between T5 and T7, and between T7, and T0, T1, and 22 (0.05). There was a significant positive correlation between the change of rate and the change of SCL-90 total score (P < 0.05); and stepwise regression results showed that the first stage, the change of interpersonal relationship entered the regression equation (P < 0.01); the second stage, the changes of interpersonal pressure and daily life distress entered the regression equation (P < 0.01) in turn, and the third stage, the interpersonal relationship pressure. The stress and learning pressure entered the regression equation in turn (P < 0.01 or 0.05). In the fourth stage, the pressure of job hunting and daily life plagued the regression equation (P < 0.01 or 0.05) in turn.
Conclusion: compared with the National Youth norm, the mental health status of the undergraduate nursing students at different time points is better. 2. At different time points, the mental health status of the nursing students is different, the undergraduate nursing students are better than the junior college students, and the mental health level of the undergraduate nursing students from two weeks to the end of the seventh month of the internship, the level of mental health is gradually rising and running.
