男性青年踝關(guān)節(jié)外側(cè)副韌帶損傷后行走步態(tài)分析

【摘要】:目的獲取青年男性踝關(guān)節(jié)外側(cè)韌帶損傷者行走時踝關(guān)節(jié)的運動學(xué)和動力學(xué)參數(shù),探討損傷者行走步態(tài)的生物力學(xué)特性。方法采用Qualisys MCU500三維運動影像捕捉系統(tǒng)與Kistler三維測力臺,對踝關(guān)節(jié)外側(cè)副韌帶損傷者與健康者各15名的行走步態(tài)進行三維同步測試。結(jié)果踝關(guān)節(jié)外側(cè)副韌帶損傷者豎直方向的地面反力變化平緩;在支撐時相中前期,損傷者前后方向的力要比健康者大;在步態(tài)周期的60%之前,損傷者左右方向的力明顯大于健康者。損傷者關(guān)節(jié)跖屈力矩變化與健康者相似,患側(cè)外翻力矩、外旋力矩最大,損傷者健側(cè)輸出功率最大。結(jié)論損傷者踝關(guān)節(jié)的穩(wěn)定性下降,行走時步態(tài)異常,為減少患側(cè)負荷,健側(cè)出現(xiàn)代償效應(yīng),足著地瞬間加速過度到垂直支撐時相。本研究為預(yù)防踝關(guān)節(jié)以及損傷后的臨床治療以及康復(fù)訓(xùn)練等提供一定的理論參考。
[Abstract]:Objective to study the biomechanical characteristics of walking gait of young male patients with lateral ligament injury of ankle joint. Methods Qualisys MCU500 three-dimensional motion image capture system and Kistler three-dimensional dynamometer were used to measure the walking gait of 15 patients with lateral collateral ligament injury and 15 healthy subjects. Results the lateral collateral ligament injury of the ankle showed a gentle change of the vertical ground reaction force; in the early phase of the support phase, the force in the front and back direction of the injured person was greater than that in the healthy one; before 60% of the gait period, the force in the front and back direction of the injured person was greater than that in the healthy one. The force on the left and right direction of the injured person was obviously greater than that of the healthy person. The change of the joint metatarsal flexion moment in the injured group was similar to that in the healthy one. The inflection moment and the external rotation moment of the injured joint were the largest and the output power of the healthy side was the largest. Conclusion the ankle stability of the injured patients decreased and the gait was abnormal during walking. In order to reduce the burden of the affected side, the healthy side had modern compensatory effect, and the foot ground was accelerated to the phase of vertical support. This study provides a theoretical reference for the prevention of ankle joint and post-injury clinical treatment and rehabilitation training.
