【摘要】：研究目的:探究清熱利濕方治療濕熱蘊結(jié)型痤瘡的療效及痤瘡患者面部特征分析。研究方法:門診選取65例濕熱蘊結(jié)型痤瘡患者及20例健康受試者,痤瘡患者隨機分為治療組33例和對照組32例,治療組予清熱利濕方中藥湯劑口服,阿達帕林凝膠、氧氟沙星凝膠外用,對照組僅外用藥膏,藥物療程為4周,患者分別于治療前、治療后4周進行皮損、癥狀積分評估。同時,痊愈病例在治療結(jié)束后2個月進行隨訪,對受試者的病情變化、復(fù)發(fā)率情況進行記錄。應(yīng)用VISIA面部圖像分析系統(tǒng)拍攝健康受試者及痤瘡患者治療前的面部照片,并記錄皮膚色素斑、皺紋、紋理、毛孔、紅色區(qū)和紫質(zhì)的絕對分值。將每位受試者所有數(shù)據(jù)輸入計算機,與健康受試者結(jié)果進行比較,經(jīng)統(tǒng)計學(xué)處理分析痤瘡患者與健康受試者皮膚特點及各項指標之間的關(guān)系。研究結(jié)果:治療4周后,治療組的皮損有效率為87.88%,對照組的皮損有效率為68.75%,治療組的癥狀皮損綜合癥候的有效率為90.91%,對照組的綜合癥候有效率為62.50%,且兩組的差異具有統(tǒng)計學(xué)差異(P0.05),對綜合癥候痊愈的患者治療結(jié)束2月后進行隨訪,治療組隨訪6例,復(fù)發(fā)1例,對照組隨訪1例,復(fù)發(fā)1例;痤瘡患者在毛孔、紫質(zhì)、紅斑區(qū)的VISIA數(shù)值均高于健康受試者,且差異具有統(tǒng)計學(xué)意義(P0.05)。結(jié)論:清熱利濕方治療濕熱蘊結(jié)型痤瘡安全有效、復(fù)發(fā)率低;痤瘡患者相較于健康受試者的不同主要表現(xiàn)在膚質(zhì)的粗糙、皮膚痤瘡丙酸桿菌的數(shù)量增多及皮膚炎癥較高、膚色的改變,綜合以上,VISIA面部圖像分析儀對面部色素斑、皺紋、紋理、紫質(zhì)、紅色區(qū)的定量分析,或許可為痤瘡的治療效果帶來一種更加客觀、直觀的臨床評價辦法。
[Abstract]:Objective: to analyze the efficacy and characteristics of facial acne patients qingrelishi side treatment of damp heat accumulation type acne. Methods: We selected 65 cases of outpatient shireyunjiexing acne patients and 20 healthy subjects, patients with acne were randomly divided into treatment group 33 cases and control group 32 cases, treatment group was treated with qingrelishi side Decoction oral, topical Adapalene Gel, Ofloxacin Gel, the control group was treated with ointment, drug treatment for 4 weeks, patients in the treatment of skin lesions before and after 4 weeks of treatment, symptom assessment. Each subject The subjects all the data into the computer, and the results were compared with healthy subjects, after statistical analysis of the relationship between acne patients and healthy subjects between the skin characteristics and indicators. Results: after 4 weeks of treatment, the lesions of the treatment group was 87.88%, the lesions of control group was 68.75%, syndrome the symptoms of skin lesions in treatment group the syndrome rate was 90.91%, the effective rate of control group was 62.50%, and the differences in the two groups had statistical differences (P0.05), follow-up of patients recovered syndrome treatment end after February, the treatment group 6 cases were followed up, 1 cases of recurrence, 1 cases of control group were followed up, recurrence in 1 cases of acne patients; in the pores, porphyrin, VISIA numerical erythema area were higher than that in healthy subjects, and the difference was statistically significant (P0.05). Conclusion: qingrelishi side is safe and effective in the treatment of damp heat accumulation type acne, acne patients compared with low recurrence rate; Different from healthy subjects is mainly manifested in the rough skin, skin inflammation and high number of skin P.acnes increased, the change of colour, based on the above, VISIA facial image analysis of facial pigmentation, wrinkles, texture, porphyrin, quantitative analysis of the red zone, or permission for the treatment of acne bring a kind of effect more objective, clinical evaluation methods are intuitive.
