雙側(cè)同期與分期非骨水泥型人工全髖關(guān)節(jié)置換術(shù)的安全性對比與分析
[Abstract]:Objective: to compare the safety and complications of bilateral uncemented total hip arthroplasty (THR) with bilateral staged total hip arthroplasty (THR). Methods: from March 2012 to March 2015, 53 patients with bilateral cement-free total hip arthroplasty were retrospectively analyzed, including 25 cases (50 hips) of bilateral simultaneous replacement and 28 cases (56 hips) of bilateral staged replacement. The changes of hemoglobin and blood cell volume, blood transfusion volume and blood transfusion rate, operation time and hospital stay, and postoperative complications were compared between the two groups. Results: the average hospitalization time of bilateral cementless total hip replacement was significantly shorter than that of bilateral staged total hip replacement [(21.32 鹵5.92) d vs (32.93 鹵11.12) dP0.05], postoperative blood transfusion volume [(447.60 鹵104.86) ml vs (245.00 鹵70.00) ml,P0.05], total blood transfusion volume [(726.20 鹵129.79) ml vs (541.57 鹵124.44) ml,P0.05] and total blood transfusion rate (76%vs 51.7% P0.05) were significantly shorter than those of bilateral stage total hip arthroplasty [(21.32 鹵5.92) d vs (, 32.93 鹵11.12) d0.05], total blood transfusion volume [(726.20 鹵129.79) ml vs (541.57 鹵124.44) ml,P0.05], and bilateral replacement volume [(726.20 鹵129.79) ml vs (541.57 鹵124.44) ml,P0.05]. Compared with bilateral stage replacement, the operation increased. The rate of complications after bilateral uncemented total hip replacement was slightly lower than that of bilateral stage replacement (8%vs 17.8%), but the difference was not statistically significant (P0.05). Conclusion: bilateral uncemented total hip arthroplasty significantly decreased the average hospitalization days, but increased the total blood transfusion volume and the total blood transfusion rate. There was no significant difference in the incidence of postoperative complications.
