【摘要】：目的:通過觀察聯(lián)合應(yīng)用三七粉與硝酸甘油治療慢性穩(wěn)定性心絞痛的臨床療效,從而為臨床提供試驗依據(jù)。方法:選取自2015年1月至2017年1月期間就診延邊大學(xué)附屬醫(yī)院門診及住院治療的60例,既符合慢性穩(wěn)定性心絞痛的西醫(yī)診斷標準,又符合中醫(yī)辨證標準的患者。將他們根據(jù)看病先后分為對照組和治療組,每組各30例。年齡和性別無顯著性差別,生理狀態(tài)基本相同。治療組給予三七粉(每次3g,每日2次)和硝酸甘油(0.5mg,必要時舌下含服)治療;對照組單獨給予硝酸甘油(0.5mg,必要時舌下含服)治療。20天為一個療程。經(jīng)過3個療程治療后對比分析兩組的中醫(yī)主要證候積分變化、心絞痛癥狀變化情況、硝酸甘油停減率、心電圖的變化,并觀察肝腎功能等安全指標。結(jié)果:1.心絞痛發(fā)作時間的比較:治療組中心絞痛持續(xù)發(fā)作時間比對照組短,P0.05,統(tǒng)計學(xué)有差異性。2.硝酸甘油停減率的情況比較分析:治療后,治療組中患者的減停率明顯優(yōu)于對照組,P0.05,統(tǒng)計學(xué)有差異性。說明治療組中患者對硝酸甘油的使用依賴性沒有對照組強。3.肝腎功能變化:治療組與對照組在治療后均無肝腎功能檢驗異常表現(xiàn),由此可見本治療方案無嚴重毒副作用和不良反應(yīng)。結(jié)論:三七粉與硝酸甘油聯(lián)用治療慢性穩(wěn)定性心絞痛,在改善心絞痛發(fā)作持續(xù)時間、硝酸甘油減停率等方面有顯著療效,同時在提高臨床綜合癥狀的療效等方面療效均大于單獨應(yīng)用硝酸甘油治療的對照組。
[Abstract]:Objective: to observe the clinical effect of combined use of Notoginseng powder and nitroglycerin in the treatment of chronic stable angina pectoris.Methods: from January 2015 to January 2017, 60 cases of outpatient and inpatient treatment in affiliated Hospital of Yanbian University were selected. The patients not only accord with the diagnostic criteria of western medicine for chronic stable angina pectoris, but also accord with the standard of TCM syndrome differentiation.They were divided into control group and treatment group, 30 cases in each group.There was no significant difference in age and sex, and the physiological state was basically the same.The treatment group was treated with Sanqi powder (3 g, twice a day) and nitroglycerin (0.5 mg, if necessary), while the control group was treated with nitroglycerin alone (0.5 mg, sublingual when necessary) for 20 days as a course of treatment.After three courses of treatment, the changes of TCM main syndromes, angina pectoris symptom, nitroglycerin reduction rate, electrocardiogram and other safety indexes were compared and analyzed.The result is 1: 1.Comparison of angina pectoris attack time: the duration of sustained angina attack in the treatment group was shorter than that in the control group (P 0.05).Comparison and analysis of the stopping and decreasing rate of nitroglycerin: after treatment, the stopping rate of the patients in the treatment group was obviously better than that in the control group (P 0.05), there was statistical difference.It showed that the patients in the treatment group were less dependent on nitroglycerin than the control group. 3. 3.The changes of liver and kidney function: there were no abnormal manifestations of liver and kidney function test in the treatment group and control group after treatment, so there were no serious side effects and adverse reactions.Conclusion: the treatment of chronic stable angina pectoris with panax notoginseng powder combined with nitroglycerin has a remarkable effect on improving the duration of angina pectoris attack and decreasing stop rate of nitroglycerin.At the same time, the therapeutic effect of nitroglycerin alone was higher than that of the control group.
