【摘要】：目的:為我院頭孢菌素類注射劑皮試的規(guī)范化管理提供依據(jù)。方法:采用回顧性分析方法,統(tǒng)計出2015年9月1日-2016年8月31日我院常用的8種頭孢菌素類[頭孢硫脒(1.0 g)、頭孢硫脒(0.5 g)、頭孢替安(1.0 g)、頭孢替安(0.5 g)、頭孢他啶(0.5 g)、頭孢米諾(0.5 g)、頭孢米諾(1.0 g)、頭孢曲松(1.0 g)]注射劑的皮試情況、皮試相關(guān)成本以及由于頭孢菌素類注射劑過敏反應所產(chǎn)生的相關(guān)費用,并對頭孢菌素類注射劑皮試成本與通過皮試減少的過敏反應成本進行比較。結(jié)果:有100 330例患者使用過上述8種頭孢菌素類注射劑,皮試率為82.49%,皮試陽性率為6.27%;皮試直接成本為3 434 411.72元,間接成本為141 985.12元;患者承擔的費用為3 162 901.44元,醫(yī)保承擔的費用為259 096.28元,全社會承擔的費用為3 576 396.84元。從全社會角度分析,8種頭孢菌素類注射劑的平均皮試成本為(447 049.61±247 395.07)元,通過皮試減少的過敏反應平均成本為(316 075.48±260 600.49)元,皮試成本明顯高于通過皮試所減少的過敏反應成本,差異有統(tǒng)計學意義(P0.05)。結(jié)論:頭孢菌素類注射劑的皮試陽性率低、成本高。國家應對此進行規(guī)范化管理,在保證用藥安全的前提下,減少患者的經(jīng)濟負擔。
[Abstract]:Objective: to provide the basis for standardized management of cephalosporin injection skin test in our hospital. Methods: retrospective analysis was used. Eight kinds of cephalosporins commonly used in our hospital from September 1, 2015 to August 31, 2016 have been collected. The results show that there are 8 kinds of cephalosporins in our hospital from September 1, 2015 to August 31, 2016. Eight kinds of cephalosporins [1. 0 g), (0. 5 g), ceftianine) (0. 5 g), ceftazidime) (0. 5 g), cefmino) (1. 0 g),) have been counted. Skin test of ceftriaxone (1.0 g) injection, The costs associated with skin tests and the costs associated with allergic reactions caused by cephalosporins injection were compared with the costs of allergic reactions reduced by skin tests. Results: the 8 kinds of cephalosporins were used in 100330 patients, the skin test rate was 82.49, the positive rate of skin test was 6.27.The direct cost of skin test was 3 434 411.72 yuan, the indirect cost was 141 985.12 yuan, the cost of skin test was 3 162 901.44 yuan, the direct cost of skin test was 3 434 411.72 yuan, the indirect cost was 141 985.12 yuan, the cost of skin test was 3 162 901.44 yuan. The cost of medical insurance is 259 096.28 yuan and that of the whole society is 3 576 396.84 yuan. The average skin test cost of 8 cephalosporins was (447 049.61 鹵247 395.07) yuan, and the average cost of allergic reaction reduced by skin test was (316 075.48 鹵260 600.49) yuan. The skin test cost was significantly higher than that by skin test. The difference was statistically significant (P0.05). Conclusion: the positive rate of skin test of cephalosporin injection is low and the cost is high. The state should standardize the management to reduce the economic burden of patients under the premise of ensuring the safety of drug use.
