【摘要】：目的了解貴州省2009-2015年國家基本公共衛(wèi)生服務(wù)項目婦幼健康管理服務(wù)(0~3歲兒童和孕產(chǎn)婦)執(zhí)行情況以及實施效果,為加快推進(jìn)基本公共衛(wèi)生服務(wù)均等化提供參考。方法采取多階分層隨機抽樣的方法在72縣(區(qū))144個鄉(xiāng)鎮(zhèn)(社區(qū))進(jìn)行問卷調(diào)查,運用熵權(quán)TOPSIS法分析孕產(chǎn)婦和3歲以下兒童健康檔案的真實性、規(guī)范性、健康管理服務(wù)滿意度等服務(wù)指標(biāo),綜合評價貴州省各年度對項目執(zhí)行的優(yōu)劣情況。采用2009-2015年全省婦幼衛(wèi)生年報數(shù)據(jù),分析孕產(chǎn)婦系統(tǒng)管理率、3歲以下兒童系統(tǒng)管理率等服務(wù)指標(biāo)以及嬰兒死亡率、孕產(chǎn)婦死亡率等核心指標(biāo)變化情況,采用卡方趨勢性檢驗統(tǒng)計推斷。結(jié)果 2012-2015年全省共調(diào)查4 400戶,有效調(diào)查問卷3 610戶,各年度TOPSIS綜合排序依次為:2015、2014、2013、2012年。0~3歲兒童、孕產(chǎn)婦真實性檔案率,規(guī)范性檔案率及服務(wù)對象滿意度均呈上升趨勢。城鄉(xiāng)嬰兒死亡率、5歲以下兒童死亡率及孕產(chǎn)婦死亡率均呈下降趨勢,且農(nóng)村顯著高于城市,孕產(chǎn)婦系統(tǒng)管理率、住院分娩率,3歲以下兒童系統(tǒng)管理率等均呈上升趨勢,且城鄉(xiāng)差距逐年縮小。結(jié)論貴州省國家基本公共衛(wèi)生服務(wù)項目實施5年來,有效提升了婦幼保健服務(wù)利用度,促進(jìn)了婦幼健康水平的提高。
[Abstract]:Objective to investigate the implementation and effect of maternal and child health management service (MCH) in the National basic Public Health Service Project (2009-2015) in Guizhou Province, so as to provide a reference for accelerating the equalization of basic public health services. Methods A questionnaire survey was conducted in 144 villages and towns (communities) in 72 counties (districts) by multi-level stratified random sampling. The authenticity and standardization of maternal and child health files under 3 years of age were analyzed by entropy weight TOPSIS method. Health management service satisfaction and other service indicators, comprehensive evaluation of the implementation of projects in Guizhou province in each year. Based on the data of the annual report on maternal and child health in the province from 2009 to 2015, the changes of service indicators, such as system management rate of pregnant women and children under 3 years of age, and core indicators such as infant mortality rate, maternal mortality rate, and so on, were analyzed. The statistical inference of chi-square trend test was used. Results A total of 4,400 households were investigated in the province from 2012 to 2015, and 3 610 effective questionnaires were investigated. The order of TOPSIS in each year was: 1: 2015 / 2014 / 2013, and in 2012, 0 ~ 3 years old children. The truthfulness file rate, normative file rate and satisfaction degree of service objects were all on the rise. The infant mortality rate and the maternal mortality rate of children under 5 years old in urban and rural areas were both decreasing, and the rural areas were significantly higher than those in urban areas. The system management rate of pregnant and lying-in women, the rate of in-hospital delivery and the system management rate of children under 3 years of age were all on the rise. And the gap between urban and rural areas narrowed year by year. Conclusion the state basic public health service project in Guizhou province has effectively promoted the utilization of maternal and child health care services and the improvement of maternal and child health level since the implementation of the basic public health service project in Guizhou province 5 years ago.
