【摘要】：近年來(lái),隨著人口密度老齡化,腦卒中已成為我國(guó)居民主要死因,是老年人健康的最主要?dú)⑹�。由于腦卒中病人多為老年患者,用藥復(fù)雜,需聯(lián)合使用多種藥物。而醫(yī)師往往只對(duì)自己專業(yè)的疾病精通,對(duì)于層出不窮的新藥,和聯(lián)合用藥方面有著不足。同時(shí)國(guó)內(nèi)的臨床藥師缺乏專屬性,沒(méi)有針對(duì)性,如何成為腦卒中疾病的臨床藥師,值得深入研究。本文通過(guò)臨床藥師參與腦卒中這一疾病的臨床治療,探索臨床藥師對(duì)腦卒中患者的藥學(xué)服務(wù)模式以及探討臨床藥師對(duì)于腦卒中疾病的工作重點(diǎn)。同時(shí)根據(jù)腦卒中疾病的特點(diǎn),建立專屬于腦卒中疾病的藥學(xué)服務(wù)流程。根據(jù)探索的藥學(xué)服務(wù)模式,重點(diǎn)對(duì)于三甲M醫(yī)院腦卒中疾病用藥情況、藥物相互作用、聯(lián)合用藥及不良反應(yīng)進(jìn)行醫(yī)囑審核、處方專項(xiàng)點(diǎn)評(píng)、病例分析。同時(shí)對(duì)在此過(guò)程中發(fā)現(xiàn)的問(wèn)題進(jìn)行了分析和建議。研究表明,臨床藥師參與腦卒中患者的藥學(xué)服務(wù),能夠有效的提高用藥合理性,減少藥物的毒副作用,優(yōu)化治療方案,從而保障了腦卒中患者用藥的安全性和有效性。
[Abstract]:In recent years, with the aging of population density, stroke has become the main cause of death among the residents in China, and is the main killer of the elderly health. Because stroke patients are mostly elderly patients, the use of drugs is complex. Need to use a variety of drugs. And doctors often only their own professional disease proficiency, for the endless emergence of new drugs, and the combination of drugs have deficiencies. At the same time, the domestic clinical pharmacists lack specificity, no targeted. How to become a clinical pharmacist of stroke disease is worthy of further study. This article is involved in the clinical treatment of stroke disease through clinical pharmacists. To explore the pharmacological care model of clinical pharmacists for stroke patients and explore the focus of clinical pharmacists on stroke diseases. At the same time according to the characteristics of stroke diseases. According to the explored pharmaceutical care model, the focus is on the drug use and drug interaction for stroke diseases in Trimethyl-M Hospital. Combined use of drugs and adverse reactions for the review of doctors' orders, prescription special review, case analysis. At the same time, the problems found in this process were analyzed and suggested. Clinical pharmacists participate in the pharmaceutical care of stroke patients, which can effectively improve the rationality of drug use, reduce the toxicity and side effects of drugs, optimize the treatment plan, and thus ensure the safety and effectiveness of drug use in stroke patients.
